HIPAA NOTICE OF PRIVACY PRACTICES

INTRODUCTION

MIX Pharmacy, Inc. is committed to protecting the privacy of your health information (PHI). This notice describes how we may use and disclose your PHI, and your rights with respect to your PHI.

OUR DUTIES

MIX Pharmacy is required by law to maintain the privacy and security of your PHI. We will only use and disclose your PHI as permitted by law, and we will promptly notify you in the event of a breach of your PHI. We are also required to provide you with this Notice of Privacy Practices and to follow the terms outlined in this notice.

 

WHAT IS PHI?

PHI is information that identifies you and can be used to contact you about your health care. This includes your name, address, phone number, date of birth, Social Security number, health insurance information, and any other information that relates to your past, present, or future physical or mental health condition.

HOW WE MAY USE AND DISCLOSE YOUR PHI

 

We may use your PHI to provide you with health care services, to bill and collect payment for those services, and to carry out our other business activities. We may also use your PHI to:

  • Contact you about your health care
  • Provide you with information about health care products and services
  • Conduct quality assurance and improvement activities
  • Comply with laws and regulations
  • Respond to law enforcement requests
  • Donate your PHI to research projects

 

MIX Pharmacy may use and disclose your PHI for the following purposes:

  1. Treatment: We may use and disclose your PHI to provide you with medical treatment and services. For example, we may disclose your PHI to your healthcare provider or to a pharmacist to ensure that you receive the appropriate medication.

  2. Payment: We may use and disclose your PHI to bill and collect payment for the treatment and services you receive. For example, we may disclose your PHI to your insurance company or a government program such as Medicare.

  3. Healthcare Operations: We may use and disclose your PHI to carry out healthcare operations, such as quality assessment and improvement activities, auditing, and compliance with laws and regulations.

  4. Marketing: We will not use or disclose your PHI for marketing purposes without your written authorization.

  5. Other Purposes: We may use or disclose your PHI for other purposes permitted by law or with your written authorization.

When do we disclose your PHI?

We may disclose your PHI to others without your consent in certain limited circumstances, such as:

  • To health care providers who are treating you
  • To health care payers who are billing for your care
  • To government agencies that are authorized to receive health information
  • To law enforcement agencies that are investigating a crime
  • In the event of a public health emergency

How do we protect your PHI?

Your Rights

You have the following rights with respect to your PHI:

  • The right to request access to your PHI
  • The right to request that we correct any inaccurate or incomplete PHI
  • The right to request that we restrict our use or disclosure of your PHI
  • The right to request that we port your PHI to another health care provider
  • The right to object to our use or disclosure of your PHI for marketing purposes
  • The right to file a complaint with the U.S. Department of Health and Human Services if you believe that we have violated your privacy rights
